When all the little details are done to a certain degree of realism and believability, it inherently makes the world more immersive, and thus the story more engaging. The littlest of details, even when seemingly having no real impact on the story or gameplay, when done to perfection, does make an impact on the story and gameplay by the aura it creates. “Unnecessary realism” should be the motto of almost every game developer; especially for RPGs targeting a more mature audience. There’s a big difference between the “popcorn” games like Call of Duty, etc., that target the lowest common denominator of gamer, and something like Kingdom Come, that is targeting fans of history, of realism, of branching storylines driven by decision making dialogue and moral choices, etc.
