If when you enter the mines, they kill the Mine Foreman, Nemoy, by pushing him, the journal says that he nearly comes down on your head. However, when you return to speak to Master Feyfar, you tell him that Nemoy got away even though he was killed by his people.
The issue may be that the person they killed isn’t marked/labeled as Nemoy. He’s labeled as a generic miner.
