This has happened almost the exact same way the last 2 sessions I’ve played this game. I get to where I’m going, do what I need to do while saving multiple times, then leave town only to be attacked by 4 heavily armored foes that I cannot outrun or defeat, die, and subsequently lose all saves when the loading fails after dying. I’m on an Xbox 1.
You need to trim your saves down. For some reason on consoles the game keeps all saves. Just trim it down to your last 10 or so, and be sure to do this frequently so the issue doesn’t arise again
