Loaded up the game after 1.4 and heard a three objective complete drum beats.
Checked my quest log and “Questions and Answers” had disappeared from the Main Quest section.
I think this needs a patch 
Tested with no mods.
Yes, there’s a bunch of people over on the reddit forum reporting this issue as well. What is the solution Warhorse? No one with this issue can continue their game.
People have said to roll back 1.4 to an earlier version and complete the “Questions and Answers” quest. But how do we do that with Steam (or with console versions for that matter)?
According to success stories on Reddit: Torrent a 3.4 release. Hack it and play past the quest. Typical. Always needing to relay on hacked games to actually play purchased content.
I think I’m going to give WH time to patch this and go play all the Witcher 3 DLC and the tons of missions I still have left to do and places explore after 440 hours of play.

I can confirm this is happening.
It auto fails the quest in my game whenever you load the file. Even going to radzig in the lookout wont solve the problem, he doesnt bring up the option to talk about the interrogation.
Same here. It’s a very odd way to fix something — just making it disappear…I was waiting for 1.3 fix, then 1.4 fix…now to 1.5, I guess?
I am playing without ANY mod an it broke the quests…
NO MODS - this is happening with the vanilla game
Same problem for me… When I start the game it automaticaly failed the quest “questions and answers” so i cannot continue the main quest…

Main quest back after the last 900mb Steam update. Going to leave it for tonight and check the forums later to see how it pans out.
Well done to WH for addressing this asap, and hope to get back to 1403 tomorrow.
1.4.1 has fixed Q&A and no cutscene for me. I can continue playing finally.
It may still crash for you in Pestilence, once you wait for the cure to work - crashes following day. With pre 1.4 saved games too.
