Let me start by saying I absolutely love this game, the part where Henry finds his parents I literally cried.
It feels very immersive and alive, Skyrim but better.
But the graphics are sometimes frustrating, npc’s walking straight through each other, horses walking through wood etc. So I decided to patch up but the problems remain. Is it normal or did I do something wrong in my patching process?
Its a great game but I can forgive little glitches like this. Really in the scheme of things its nothing.
I just started the Nightingale patrol mission, I follow him around and suddenly the guy falls through the road.
He just disappeared and now I cannot complete.
So I have to restart this part. What are these patches for, seriously?

On PC. I can play the game on very high settings.
NPC falling to ground was old bug, I thought it is fixed 
The one thing that made me cringe was when Henry head was missing in cutscenes because I wore full head helmet all the time, especially at the end, I was frustrated, I hope they fixed this.
Maybe I had to start a new game after installing the patches?
Always, if not, load before you triggered any quest line.
Ok, I started new game and Henry already has a beard in the first conversation with his mother… That’s not normal is it?

This game, despite its greatness, is really starting to piss me off.
All this talk about immersive and realism and then you got guards falling through roads and people walking straight through each other. Also in cutscenes the people get stuck in between rocks, stairs etc.
Shouldn’t that be part of a patch fix?
Report it to support then, maybe they fix it. I did this before and they fixed one of my bugs. I did not have any NPCs falling through the ground as you but it was old bug known in the community. People walking through each other is lesser flaw, however, they should avoid each other. There is another widespread visual bug, missing heads of people in Rattay and even other places if your PC can not work fast enough.
The missing heads don’t seem to bother me, sometimes certain particles aren’t loaded as fast as I would want but I can live with that. I already sent an email to support, and am awaiting further communication.
Is there a list of all patches and the changes they entail?
Not seen those bugs, but I know The Witcher 3 had them and everyone called it GOTY still.
Bug is an issue which causes your game to crash or certain parts of game not working as intended. If your game works and nothing stops you to progress, then you are bugless.
