It seems that this Achievement is affected by a ceneral counter? I am half through the mainstory (just killed runt) so only sidequests available to this point. Just finished “From the Ashes” and got it… lol
Hm, I also feel it is bugged - I did not complete several quests and I still got this ! I think it should be given to people who complete 99% of all quests…
