I got stuck in the tiny creek next to the house of miller Woyceck. It is impossible to jump out, no way to get of the water.
I got stuck at Woyceck’s as well! Except I was riding really fast along the path and ended up half-climbing up the barn/shed thing parallel to his house. Anyway I got stuck there. Really annoying cos I had spent ages hunting and selling the meat and I had to reload. Ah well!

Just try Fast travel to get out.

Yea, but that can’t really be the solution. 
You can also try to call your horse and try to jump out of it.
Are you overburdened? This might be why you can’t jump anymore. If so, drop some loot.
Have you tried “Save and Exit” and reload?
If he was overburden he would not be on the horse in the first place, But yes this game has a lot of problems
