Can you decribe what happens? I mean what are the npcs saying to you?
And if you go into the player statistics area, and look at the tab for Rep, highlight the town where you have the low rep and expand to see what portion of that town actually dislikes you - what do you see?
maybe you’re dirty? Get some dialogue and specific example (for example, talk to multiple named people, like traders or people with names not villagers or vagabonds) and tell us what they say to you exactly. What town you are in, and what mission you are on. There are quite a few bugs in this game, the more specific you can be the better people will be able to help you. Good luck!
Yup, there’s a bug.
For me it was Sasau, I had -60 reputation there before I even discovered it.
That shouldn’t be a bug. You just did something bad to someone who is connected with Sasau. 
-60 is a lot though, if you kill someone somewhere it goes down by 3 or so.
So -60 would be like murdering the entire city and t-bagging their corpses.
And that has gradually gone down to -100 now.
And I don’t even kill anyone or steal anything, I act the exact same as anywhere else, and my Rattay rep is literally 100. So no, it’s definitely a bug.

only thing i can think of is that my fisrt quest for the miller, I walked into Executioner’s yard and i was just moseying along and dogs were barking (i wasn’t crouching or doing anything) and he just sprinted across his property in his under-roos and started a fight. We fistfought, I let him go unconditionally, and when I finally made it to Rattay everyone looks for 200+ grochen for a lousy chat lol
Also somehow my Rattay Solider level is in the 20’s…no idea how that jumped down so far when i haven’t interacted with them outside of working with the bailiff
Or maybe the OP had a wayfarer dead body encounter where an NPC conveniently shows up to accuse you of murder. And then you let them actually get away aka beat you in a foot race to the nearest town…
